The Salt Substitute and Stroke Study is one of the largest randomized trials directly testing whether changing the composition of household salt affects clinical outcomes.

Study Design
Villages were cluster-randomized to salt substitute or regular salt.
Participants had a prior stroke or were aged 60 years or older and had hypertension.
Main Outcomes
The NEJM paper reported a lower rate of fatal or nonfatal stroke in the salt-substitute group than in the regular-salt group.
Major cardiovascular events and all-cause death were also reported at lower rates.
Safety Outcome
Hyperkalemia was monitored as a safety outcome and the trial reported no apparent increase in serious adverse effects.
The findings apply to the specific population, formulation and trial conditions studied.
