The Tumbes, Peru study is notable for applying salt substitution at community scale rather than only to people with diagnosed hypertension, using a stepped-wedge cluster-randomized design.

2,376 Participants and Six Villages
A total of 2,376 people from six villages participated.
Regular household salt was retrieved and replaced free of charge with a blend containing 75% NaCl and 25% KCl.
Blood Pressure
Fully adjusted analyses reported average reductions of 1.29 mmHg systolic and 0.76 mmHg diastolic blood pressure.
Among participants without hypertension at baseline, the incidence of new hypertension was reported to be lower.
Potassium Finding
Twenty-four-hour urine showed no clear difference in sodium but higher potassium at the end of the intervention.
The study is an important example of salt substitution implemented under real community conditions.
