The Tumbes study in Peru used a stepped-wedge cluster-randomized design in which villages crossed from regular salt to the salt substitute sequentially over time rather than all at once.
More information on this topic is available in the Scientific Salt Research section.
Why Villages Were the Clusters
Because the intervention operated through households and local food environments rather than isolated individuals, randomization occurred at village level.
This helps reduce contamination between different salt strategies within the same community.
Stepped-Wedge Logic
Villages moved into the intervention at different times, creating both control and intervention periods during rollout.
The design is useful for introducing large community interventions gradually when resources are limited.
Real-World Implementation
Regular household salt was retrieved and replaced free of charge by a 75% NaCl/25% KCl product.
The study therefore targeted everyday household use rather than only a laboratory or clinic setting.
